Boarding Form

Please fill this out prior to your pet’s boarding reservation.

  • Trail walks are available (weather permitting) weekdays except for holidays. Pets are harnessed to provide safety. Please fill out the "Trail Walk Form" if a walk is requested.
  • Personal Items will be labeled. We provide necessities such as bowls, bedding, toys, treats. We recommend keeping personal items minimal. We will NOT be responsible for items damaged or left behind.
  • We provide Purina EN and Science Diet I/D to boarding pets. If they have sensitive stomachs, we recommend bringing their own food. Please note how much you feed, how often, and what you feed if providing your own food.
  • All pets boarding at Vintage Heights Veterinary Hospital are required to be currently vaccinated and have a current annual Heartworm Check as well an an annual fecal check. Rabies, Distemper/Parvo and Bordetella are required for dogs. If your dog is vaccinated at another facility, you must provide documentation from a veterinarian. If you do not provide documentation, you will be charged for required vaccinations and service.
  • All pets boarding at Vintage Heights Veterinary Hospital are required to be currently vaccinated. Rabies and Distemper and an annual Fecal Check are required for cats. If your cat is vaccinated at another facility, you must provide documentation from a veterinarian. If you do not provide documentation, you will be charged for required vaccinations and service.
  • Please list any medications your pet is on, what the dosage is, how often they're given and when last given. Pet's requiring medication will be charged an administration fee of $3.40 per administration. Additional administer medication form required to be completed.
  • If your pet is admitted to boarding and has fleas/ticks, your pet WILL be treated with a preventative at your expense for a fee of $29.50. This is necessary to prevent the spread of fleas/ticks in our hospital and boarding facility. If your pet needs preventative upon arrival, please note below.
  • If your pet develops diarrhea related to the stress of boarding (most common), we will treat accordingly at your expense. If your pet develops an urgent medical condition while under our care, we will make every attempt to contact you at the emergency number you have provided. If we cannot reach you and your pet requires immediate veterinary care, appropriate treatment will be administered and all fees will be the owner's responsibility. After treatments have bee initiated, we will continue to try to make contact with you at the emergency number provided by you. We do NOT offer pick up times during closed business hours.
